504 Coordinator

You know which students have plans. You do not know which plans are being followed, and you find out the hard way, at a parent meeting or when a grade drops.

Three screens that move a student from referral to a signed plan to a review date, and show you whether the accommodations are actually happening.

How you use it

01

Referral and eligibility

Referral intake, evaluation checklist and eligibility decision in one workflow, with the sources considered, the team present and the rationale recorded as you go.

Answers: Where is this student in the process, and what is the next step.
02

Plan drafting and teacher roster

Accommodations, aids and services drafted from the evaluation record, including behavioral and health plans for ADHD, anxiety and medical conditions. The moment the plan is signed, it appears on the roster of every teacher who has the student.

Answers: What does this student get, and does every teacher already know it.
03

Delivery and review calendar

Teachers log accommodations as they deliver them. You see delivery against the plan, the student's attendance and grade trend next to it, and a calendar of annual reviews with reevaluation triggers when placement or performance changes.

Answers: Is the plan working, and which reviews are due this month.

What you can prove

Every student with a plan, every accommodation on that plan, and whether it was delivered. When a parent asks how the plan is going, you answer with a screen, not a memory.
